What the old astrologers actually looked at

The historical practice was narrower than the modern label suggests. Four things recur in the sources.

The sixth house. Firmicus Maternus assigns it illness and the cause of faults and health, and al-Qabisi gives it illness, slaves, beasts and prison. Dorotheus calls the sixth and twelfth the worst of the worst.
The planets and their parts. Ptolemy judges hurts to the body from the angles on the horizon and from malefics aspecting them, using his list of planetary body parts. See Planets and the Body.
The sign and its region. The body map assigns each sign to a region, and al-Qabisi adds a limb for each planet in each sign. See Melothesia.
The temperament. The planets and signs were classed by hot, cold, moist and dry, the qualities of the humors, which was how a physician of the time thought about health.

None of this is a method for diagnosing a modern illness.

Length of life and the hyleg

Ptolemy works toward length of life and the condition of the body in Book III of the Tetrabiblos, after parents, siblings and twins. The key idea is a significator of life, a point in the chart judged to carry the native's vitality. He examines four candidates, the Sun, the Moon, the ascendant and the Part of Fortune, and the medieval Arabic and Latin tradition calls the chosen point the hyleg. Dorotheus checks the three triplicity lords of the ascendant for survival and strength in the first book of the Carmen.

The Great Almanac treats these life-length methods as history. They are no basis for any prediction about a person, and a question of that kind belongs with a doctor, not a chart.

The four humors and the elements

The medicine behind the astrology was the doctrine of the four humors, and the signs carry it. Abu Ma'shar's Abbreviation classes the fire signs as hot, dry and choleric, the earth signs as cold, dry and melancholic, the air signs as hot, moist and sanguine, and the water signs as cold, moist and phlegmatic, and gives each sign a taste.

That scheme is why the tradition can feel like a coherent system. It is a coherent system of its own time, and modern medicine does not use it.
